Except in life-threatening emergencies, or unless required by federal, state, or local law(s), your medical information will not be released without your permission. On occasion, Campus Health receives telephone calls from parents about your medical care. If you are 18 years of age or older, Campus Health is unable to release any medical information to your parents without your explicit verbal or written permission. We do not allow for students to sign a “blanket” authorization form. What you might want to release to a third party now, you might not want to release at a later date. You will need to sign a release form each time you want new information released from your records.
There may be times when the transfer of medical care to another health care institution or organization is necessary. In order for medical information to be released, a written release must be signed by the requesting student.
You can also view your personal health records on the Healthy Heels Portal. Your login will always be your UNC ONYEN and ONYEN password.
Release of Information
- You need to complete a Campus Health Release of Information Form before we can release any information.
- Complete all sections of the form – requests will not be processed if the form is incomplete.
- *Please note we cannot accept an electronic signature on Release of Information forms, all forms must have an original signature before the request can be processed.
- Requests for your records can take between 7-10 business days to process. If you only need copies of your immunization records, you can pick up those records same day by visiting the Health Information Management Department located on the second floor of the building.
Accessing Radiology Reports and Images
All radiology reports and images for X-rays that are performed at Campus Health will be available through My UNC Chart . If you were referred for radiology studies to a facility other than UNC or its affiliates, you will need to contact that facility for accessing your reports and images.
If you do not already have a My UNC Chart account, you can create an account by visiting My UNC Chart Account Signup.

Once you have logged in to your My UNC Chart, follow the steps below to view your radiology reports and images:
- Select Test Results
- Select Radiology Study
- Select Linked Information to view images

Sharing Radiology Reports and Images to Outside Providers
To request radiology reports and images for X-rays performed at Campus Health to be sent to an outside provider who accepts Power Shared Images, please complete the highlighted portions of the Power Share Release Form and email to [email protected]. Please contact your outside provider first and inquire if they accept Power Shared images.
