Exemptions for State Required Immunizations

Medical Exemptions

Medical exemptions for required immunizations will only be granted upon submission of either a Medical Exemption Statement (DHHS 3987) or Physicians Request for Medical Exemption (DHHS 3995).  Both of these exemptions require a signature by a Physician in North Carolina.

Religious Exemptions

Religious exemptions for required immunizations will only be granted upon submission of a statement of a bona fide religious objection to immunizations by the student (or by a legal guardian if the student is less than 18 years of age). The document must include the name, date of birth, and UNC PID# of the student for whom the exemption is being requested. Statements of bona fide religious objection to immunizations do not need to be notarized, signed by a religious leader, or prepared by an attorney. If needed, students can submit the Campus Health Religious Exemption form in lieu of a written statement. Please note that North Carolina does not accept exemptions for the case of a personal belief or philosophy of the student or legal guardian, the exemption must be founded upon a bona fide religious belief.

Medical and religious exemption form(s) must be submitted via the students ConnectCarolina To Do list.
