North Carolina Required Immunization Information
REQUIREMENTS
All new and transfer students are required to submit documentation of state required immunizations. Students who were previously enrolled at UNC – Chapel Hill are not required to resubmit immunization documentation.
The general deadline for submission of immunization records is:
- June 15 for Fall admissions
- December 15 for Spring admissions
All immunization records must be submitted by the first day of classes.
Per North Carolina state law, students will be withdrawn from the University 30 days after classes begin if immunization requirements have not been met.
How to Submit Immunization Records
To access the Immunization Form and to submit your immunization records:
- Login to your Student Center page in ConnectCarolina .
- Click on the To-Do Lists.
- Under the To Do List column, click on General Items, Immunizations Required, and follow the directions for accessing and submitting immunization records via ConnectCarolina. This To Do List item will only be available once you have been matriculated into the University.
All students must submit immunization records via the ConnectCarolina To Do List item. Carolina will not accept hard copy, faxed, or emailed forms.
- Students can submit immunization records provided by a local medical providers office, high school, state immunization registry, etc in lieu of using the UNC Immunization Form.
- All immunization records submitted must be in English and contain the students demographic information (name, date of birth, etc.) to be accepted. If records are not in English, the UNC Immunization form must be used. The form will need to be signed/dated by a clinician who can verify the immunization dates on the form.

Immunization Requirements by Vaccine
Diptheria/Tetanus/Pertussis
Three (3) doses
- At least one of the three doses must be the Tdap booster (tetanus, diptheria, and pertussis), given between 10 and 12 years of age or when entering the 7th grade.
- If the student does not have this booster dose, they must receive the booster before matriculation to the University.
Polio
Three (3) doses
- An individual attending school who has attained their 18th birthday is not required to receive polio vaccine.
Measles
Two (2) doses (or 2 MMR doses)
- Both doses must have been given on or after the first birthday and doses must be separated by at least 28 days.
- Antibody testing confirming immunity to measles will be accepted In lieu of vaccine (must submit a copy of the lab results)
- The second dose of measles is not required if the student:
- Entered college or university for the first time before July 1, 1994
- Was born before 1957
Mumps
Two (2) doses (or 2 MMR doses)
- Both doses must have been given on or after the first birthday.
- Antibody testing confirming immunity to mumps will be accepted In lieu of vaccine (must submit a copy of the lab results)
- The second dose of mumps is not required if the student:
- Entered college or university for the first time before July 1, 2008
- Was born before 1957
Ruebella
One (1) dose (or an MMR dose)
- Must have been given on or after the first birthday
- Antibody testing confirming immunity to rubella will be accepted In lieu of vaccine (must submit a copy of the lab results)
Hepatitis B
Three (3) doses
- A two-dose series of Heplisav-B is acceptable
- Antibody testing for Hepatitis B will not be accepted. Students must show documentation of a completed Hepatitis B vaccine series.
- Hepatitis B vaccine is not required if a student was born before July 1, 1994.
Varicella
One (1) dose
- Documentation from a clinician verifying varicella disease is accepted in lieu of vaccine dates.
- Antibody testing confirming immunity to varicella will be accepted In lieu of vaccine (must submit a copy of the lab results)
- Varicella vaccine or proof of immunity is not required if a student was born before April 1, 2001.
Meningococcal Conjugate
One (1) dose
- Must have been given on or after the 16th birthday.
- Vaccine brands that satisfy this requirement are: Menactra, Menveo, MenQuadfi, Nimenrix, and Penbraya.
- Meningococcal B vaccines (Bexsero & Trumenba) will NOT meet this requirement.
- Meningococcal C vaccines will NOT meet this requirement.
- Meningococcal is not required if a student was born before January 1, 2003.
Tuberculosis Testing
- TB Testing is REQUIRED for students from countries with an increased incidence of Tuberculosis (TB).
- TB testing is RECOMMENDED for students who have risk factors that increase their likelihood of exposure to tuberculosis (TB).

The NC Immunization Branch website has additional specific immunization age requirements for matriculating students. Also note there are possible exemptions to immunizations.
Health Science Students
All incoming Health Sciences students must comply with the North Carolina immunization requirements for newly enrolled students. In addition to these state requirements, your academic program may require additional immunizations or health documentation.
It is your responsibility to submit all required immunization records directly to your program. Please keep a copy of your records, as the University does not automatically forward immunization information to individual programs.
