• NOTE! If you are ill and need to be seen that day, call as early as possible in the day to make your appointment.
  

• Download the Diabetes Questionnaire, fill it out and bring it with you to the appointment.
• Request that your doctor send your latest diabetes records to Dr. Vimmerstedt.
• Incoming (New) Students with Diabetes-download the Diabetes Questionnaire, fill it out and send to the address on the form (whether or not you plan to receive your diabetes care at Campus Health Services).
