Safer Sex Materials Request Form

After requesting, safer sex materials will be available for pickup at the front desk of Counseling and Psychological Services (3rd floor of Campus Health Services in the James A. Taylor Building) within approximately three business days. A member of Campus Health Services will contact you to confirm that your request was received and is ready for pick-up.

NOTE: If you are requesting supplies for a specific program, please request them at least three business days prior to the event.
